Fire Public Event Request Form
Above is the Hawaii Fire Department’s Public Event Request Form. If you wish to request members of the fire department to attend your event, have a fire truck show up at your school during fire prevention week, have a representative attend your job/career fair or any other event, please use the form above. Please submit this request form a minimum of two weeks prior to your scheduled event. After your submittal is reviewed, you will be notified by email if HFD will be able to assist you with your request.

Please feel free to print out as many copies of this form as you would like. After filling out this form, please leave it in a well accessible area. We recommend keeping it on your refrigerator or somewhere else where it will be highly visible to EMS responders when they arrive.
Assembly
An annual permit is required for all assemblies with an occupant load of 300 or greater. A fee of $200.00 shall be assessed for each 20.1.1 permit.
LPG / AST / UST
Permit Application: LPG / Aboveground Storage Tank (AST) / Underground Storage Tank (UST)
A permit is required for all LPG tank installations equal or greater than 125 gallons.
A permit is required for installation/removal of all non-LPG tanks containing flammable or combustible liquids in excess of 60 gallons. A one-time fee of $200.00 shall be assessed for each 66.1.5 permit.
Flammable Finishes Spray Booth, Area, or Room

An annual permit is required for all businesses utilizing the spray application of flammable and combustible finishes. An annual fee of $200.00 shall be assessed for each 43.1.1.4 permit.
Fair / Market / Tents & Temporary Structures
Permit Application: Fair / Market / Tents & Temporary Structures
A biannual permit is required for all Open Markets, Flea Markets, and Farmer’s Markets. A fee of $100.00 shall be assessed for each 10.15.1 permit, good for a six month period. April 1 - September 30 and October 1 - March 31.
A permit is required for events utilizing tent and temporary structures greater than 700 square feet. A one-time fee of $50.00 shall be assessed for each tent/temporary structure exceeding 700 square feet in a 25.1.2 permit. Exemptions: Tents used on private property and not open to the general public.
A permit is required for each Fair or Carnival. A fee of $200.00 shall be assessed for each 10.15.1 permit.
